Application

To apply for a place at DASH please complete the attached Waiting list application form or contact us directly.

Once your Application form has been received you will be contacted by DASH and informed of the availability of places to meet your needs. If places are not available you will be placed on the waitlist and contacted when a place becomes available.

Once your place is confirmed you will be required to complete a registration form and parent/carer contract.

Once you have registered you will need to complete a booking form which will be provided to you for completion each half term.
